Aster, known for his short Horror films, particularly The Strange Thing About the Johnsons, caught the attention of A24, which greenlit Hereditary as his first full-length feature. Filming took place in Utah in 2017, with many indoor scenes shot on custom-built sets designed to evoke a dollhouse aesthetic. Annie discovers her mother Ellen’s old belongings and finds a photo album depicting Ellen as “Queen Leigh,” the leader of a coven, with Joan as one of her followers. Another book details the demon king Paimon, who desires to inhabit a male host, promising wealth and rewards to the summoner. In the attic, Annie is horrified to find Ellen’s rotting, headless corpse along with occult runes drawn in blood. Meanwhile, at school, Peter encounters Joan, who attempts to expel his spirit for the demon king. During class, he is suddenly taken over by an unseen force and slams his head against his desk, injuring himself. Annie realizes the connection between her family and Charlie’s sketchbook and pleads with Steve to burn it, unable to end her own life. When he refuses, believing she has lost her mind, she throws the book into the fire, only for Steve to spontaneously burst into flames. In shock, Annie’s expression turns blank as she becomes possessed by the same force that overtook Peter.
As cult members gather inside and around the house, Peter wakes after dark to find his father’s charred body. He then sees one of the cult members lurking nearby. A now-possessed Annie pursues him through the house, leading him to the attic where she beheads herself with piano wire. Traumatized, Peter jumps from the attic window but falls to his death. A glowing orb reanimates his body, and he begins to mimic Charlie’s mannerisms. He follows Annie’s floating headless corpse to Charlie’s treehouse, where Joan and the other cult members worship a mannequin adorned with Charlie’s crowned severed head. Joan removes the crown and places it on Peter’s head, addressing him as Charlie. She declares that Charlie is now Paimon, having “corrected his female body” and granted him his preferred male host. The coven then hails Peter as King Paimon.
